You know how people would say, "You're never fully dressed without a smile"? Well perhaps he may have taken that too literally.
Besides, a smile is nothing short from a piece of clothing. Something that you put on to get ready for the day to not seem like a sad, lonely freak. Something you can simply wear to prevent people from ever seeing what was underneath.

Akechi bared his teeth, eyes in shadow. “Nobody has ever known what to do with me,” he said, voice dripping acid.
“Then, this should be refreshing for you,” he said, delighting in the way that Akechi’s gaze sharpened as Naoto plucked a nonexistent piece of lint from his coat. “You’re coming with me. To Inaba.”
At the end of P5R, the world forgets Akechi.
