At first, it seems as if the hex bag hasn't worked.
Dean doesn't feel any ill effects. He doesn't spontaneously combust or break out in boils, so they chalk it up to the witch being off her game. They're wrong however. It takes a little time to work, but the spell is stranger than anything the Winchesters have previously encounter.
Suddenly, Dean is sporting tentacles, and things only get weirder from there.

Dean’s sitting at the kitchen table with Sam when his phone rings. Jody’s name flashes across the screen and he considers not picking up. It’s not like Dean doesn’t want to talk to Jody, it's just that he can’t bear talking to anyone these days.
“Hi, Jody,” Sam says.
“Howdy, boys.” She asks them to take over a case. Something about werewolves. Dean doubts they will, Sam hasn’t let him pick up a gun in months, not since their last hunt.
Jody’s voice cuts through the phone. “Alright, if you boys don’t wanna take the case, I’ll see if Castiel will, but I think he’s helping Claire in Colorado–”
Dean doesn’t hear a damn thing after that, and by the looks of it, Sam doesn’t either. For the first time in days, Dean opens his mouth and speaks. “Cas is alive?”
Jody’s voice is quiet on the other end. “He didn’t tell you?”

Summary
In a bid for control of Heaven, the archangel Raphael attacks Castiel and his allies, leaving Balthazar no choice but to send the Winchesters to an alternate universe for the safety of the world with a key that opens a lock containing all of Heaven’s stolen weapons. Sam and Dean discover that in this alternate reality, they are actors on a TV show called Supernatural. Even more of a shock, Dean learns that his alternate universe self is married to a man, a man who happens to be the mirror image of his best friend. With their world once again on the verge of the apocalypse, the Winchester brothers race to find a way back to their own universe before Raphael can send his minions after them and Dean tries to come to terms with the fact that maybe his angel just might mean a little more to him than Dean previously thought.
