That’s a problem Raph had. He felt every minor thing so strongly that he couldn’t even stay still half the time. Every single emotion, positive or negative, felt as though it was freezing him up head to toe and stopping his mind and heartbeat alike, or as though someone had blinded him, ignited his heart, bones and flesh and with that, it was only a matter of time until the fire grew and burned everything around it, including Raph.

After Splinter's death, the four brothers couldn't quite fit in their predestined roles as well as they could before.
Raph had decided to be the first one to feign normalcy; it's not his fault he didn't know that it would also trigger his downfall. Well, he guessed it was better for him to take the blunt of it as long as his brothers didn't have to deal with him. If he couldn’t protect his father, then he could at least protect his brothers, even if only mentally.

“Why’re you here?” he asked. Dean stopped short, frowning and lowering the pills in a mix of hurt and confusion.
“Because I’m your brother,” Dean said firmly. “Why wouldn’t I be here?”
Sam frowned at that, looking almost confused. Dean’s heart pounded with worry.
“But… you were with the angels. I… I left,” Sam said, voice almost hesitant. Dean sighed, understanding washing through him.
“We’re at Bobby’s house, Sammy,” he said, motioning to the familiar room. “We’re together. It’s okay. Now will you please take this medicine-”
But Sam was already moving, pushing himself away from Dean and shaking his head hard enough that he swayed, almost losing his balance. Dean reached out frantically to steady him, but Sam whimpered at his touch and pulled away like he’d been burned.

Or, Sam gets sick for the first time since the apocalypse - since his and Dean's fight - and in his fevered state, some hidden thoughts and worries of his slip out. Dean makes sure to shut down his doubt quick.

He felt pure anger and hatred bloom in his chest as he got a closer look at the blooming purple bruise on Sam’s cheekbone.
“It’s nothing,” Sam said, pulling away from Dean and trying to brush him off. Dean didn’t let him, only stepping closer.
