Dumbledore was never informed about the change in time and location, so Harry lost his trial summer before fifth year. In a bid to make an example of him to Dumbledore, Fudge sentences him to twenty years in Azkaban for breaking the statute of secrecy, nevermind the only witness was his blood relative who already knew about magic existing.
When Voldemort finally revealed himself, Fudge, urged on by Umbridge, ordered that Harry receive the dementors kiss, in an attempt to cover his misdeed. Unfortunately for them, Harry was placed in a different part of the prison after Voldemort broke his death eaters out, so the wrong person is kissed.
At the end of the twenty years, prisoner 12357AZK is released, and Harry emerges into a world completely different from the one he'd left.
Voldemort had taken over the ministry, Umbridge had implemented her muggleborn registration program, Hogwarts had fallen, and Dumbledore had forced Neville into the role of chosen one.
After Dumbledore learned Harry had been kissed, he collected the horcruxes himself, and destroyed them, but left the task of defeating Voldemort solely on Neville shoulders after succumbing to temptation and donning the ring.
The war lasted an entire decade before Neville, with the help of what remained of the order, finally managed to kill the bastard, only to discover, he wasn't really dead.
For the last ten years, the few living members of the Order had been fighting to kill Voldemort over and over again, but also stay under the radar of the muggles who had finally discovered magic existed due to the countless breaches of the statute thanks to the war.
Harry, mind somehow still partially intact manages to make his way to Grimauld place, albeit after several months, due to his lack of wand and feeble state.
The only people left hiding there are Sirius, Snape, Bill and Fred Wesley and Hermione. Everyone else was either dead or had disappeared.
The only one who did not seem completely surprised to see Harry, was Snape, who had always suspected Harry was alive due to the vow he took to protect the boy, which would have potentially killed him had Harry been truly dead. He'd actually come to the conclusion, after the boy having been kissed, that someone had stolen his body and was keeping it alive in hopes of finding a way to revive him one day.
A year after Harry shows up at Grimauld, he is undergoing a ritual, cooked up by Hermione and Bill, and thanks to Snapes knowledge of the dark arts, Sirius' family library, and Fred's ingenuity and determination, Harry is being sent back in time.
Well, his soul is at least, with all his memories, and thousands of borrowed ones he'd been made to view in a pensive.
He'd argued with them constantly that he was the last choice of people to send back, that it should be Snape, or Bill, since both of them would have more time and authority with which to change things well enough to make sure the future they were living never happened.
Hermione was the one to explain that it has to be Harry, due to the prophesy marking him as the chosen one, and the power the dark Lord had accidentally transferred to him when he got his scar, he is the only one who could potentially survive being thrust back in time the way they were trying.
So, the night if Samhein 2015, Harry James Potter bled out in a ritual circle, his blood powering the runes beneath him, which was amplified by the chanting surrounding him. As midnight struck, there was a flash of light, and Harry was gone. Exactly three hours later so was everyone and everything in that thread of the universe.
